The loss of the Orbital Antares launch vehicle and of SpaceShipTwo in the course of less than a week is certainly proof that spaceflight is complex and dangerous. New entrants into the spaceflight industry are destined to suffer “infant mortality”-related failures and it is time to bring the issue out of the closet.
